Tests « essential from an epidemiological point of view »

« The serological tests are interesting because they shed light on how the infection was able to spread in our region, » details the Montpellier University Hospital. These tests make it possible to retrospectively know what has happened in a population, by targeting groups of people: caregivers, residents of nursing homes, general population. This knowledge is essential from an epidemiological point of view. And to know for example since when, precisely, the virus circulates in Occitanie and more generally in France.

Serological tests are techniques for detecting antibodies. They make it possible to assess the proportion of people who have contracted the virus in a given population. They are different from virological tests (PCR), which determine whether a person is affected by the virus. « It is a complementary method to PCR particularly useful for hospital patients and consultants when virological tests are negative while the clinical picture or pulmonary imaging are suggestive », specifies the Montpellier University Hospital. IDvet explains that its serological test can detect antibodies against Covid ‐ 19 with a specificity of 99.9%.

Recently, Dominique Grandjean, who heads the veterinary service of the Pompiers de Paris, has started a test to find out if dogs can detect the coronavirus. No treatment has yet been administered to prevent odor interference.

A practical and inexpensive screening method

Viral pathologies most often present an odor that trained dogs are able to recognize. If it turns out that this is also the case with Covid-19, it is possible to train canids to detect this disease. In fact, British researchers are already exploring this avenue.

In France, Professor Grandjean has carried out similar tests. He believes that dogs can be trained in 5-10 days. As a result, the results will be available in several weeks and are likely to be conclusive.

The Hexagon already has nearly 3,000 sniffer dogs. They could be used to identify individuals infected with the coronavirus in public places such as airports and train stations.

Professor Grandjean specifies:

It would be an additional screening tool, not the only one of course, but given the number of tests that will have to be performed after the deconfinement, we may be very happy to have these dogs. We will not have them for May 11, but we can hope, if it works, to make them operational in the weeks that follow.

The first advantage of this solution lies in its cost. Indeed, it does not require massive investments. Otherwise, canine screening is particularly safe. In the case of cancers, for example, reliability is close to 100%. However, it is only about 70% with PCR tests used in Covid-19 research.

A promising track

The first test took place in Paris during the last week of April 2020. It involved 4 fire dogs already trained in olfactory screening. In particular, they have been trained to search for people and to identify and memorize the smell of drugs or explosives. Faced with familiar scents, they start to bark or lie down.

Since May 1, the experiment has been carried out with other canines working with firefighters and gendarmes in Ajaccio (Corse-du-Sud). Concretely, this involves adding the smell of the coronavirus to those with which these animals are already accustomed. Professor Grandjean explains:

A dog that is looking for explosives is already initiated into forty molecules, if we add the Covid, it will ultimately only be a 41st for him to assimilate, it’s in his ropes, we don’t no need to start all the memorization work from the beginning […].

The training is therefore very fast, as the specialist adds. However, SARS-CoV-2 must involve specific molecules which will be found in sweat. For the time being, the experts are still at the experimental stage. As Professor Grandjean points out:

We presume, but even if it seems logical, we are not sure of anything yet, because there have been no publications on it.

In any case, these tests attract attention. Belgium, the United Arab Emirates and Morocco have already approached Professor Grandjean to find out more about his project.

What is a spikelet?

Spikelets constitute the inflorescence of the large plant family of grasses. They are made up of glumes. These are weeds (wild oats, wild oats …) that are found everywhere.

The spikelet is the plant seed which, at a certain level of desiccation (dryness), detaches from the stem (it is the ginning) and is disseminated by the wind or the coat of the animals.

The spikelet is therefore dangerous when the desiccation of the grasses is significant, from June to September.

In veterinary medicine, there is a tendency to group any plant foreign body under the term spikelet.

How can it affect the dog?

When the spikelet is detached from its plant, it can fall on a dog’s coat and then risks progressing either through natural orifices (nostrils, ear canals, etc.) or through folds of skin. The spikelet then becomes a foreign body for the dog.

The spikelet peduncle is particularly traumatic and can puncture external tissues or mucous membranes. It is a very invasive foreign body.

What are the symptoms ?

The damage caused by the spikelet in dogs depends on its location and its stage of progression.

Three major locations are listed: the ear canal, the interdigitated spaces (between the fingers) and the eyes. But all other locations are possible: anal glands, nasal cavities, vulva, trachea …

Spikelets can be responsible for mild to severe disorders. Sometimes it is just a nuisance for the dog but it can also cause a significant local inflammatory reaction, even secondary lesions.

The ear canal is the most common location of the spikelet in dogs. It causes typical reactions: the animal shakes its head, moans if touched on the ear because the pain is severe. Then appear signs of otitis: purulent discharge, tilted head … Without intervention, the spikelet can continue to progress in the ear canal and perforate the eardrum.

When the spikelet is positioned between the fingers, it causes untimely licking of the tip of the leg and can then generate an abscess. After a few days, the dog will show lameness.

In the event of ocular localization, the animal will present signs of pain and discomfort (discharge, intense blinking of the eyelids …). Untreated, the spikelet can cause a corneal ulcer.

How to solve this problem ?

The management of a spikelet requires the intervention of a veterinarian.

When the spikelet has entered the ear canal, the treatment will consist of removing it using an otoscope and forceps. The veterinarian will then look for other spikelets, assess the damage to the ear canal and treat the reaction otitis.

Interdigitated spikelets may require a little surgery to manage the fistulas they have generated.

Likewise, spikelets located at eye level may require general anesthesia for their removal.

How to prevent spikelet « attacks »?

Several risk factors are known for this problem, in particular the breed of the dog and more particularly the nature of the coat and the wearing of the ears. Cocker spaniel, English springer, setters, bobtail, briard … are thus predisposed.

The way of life also plays a role with an influence of the activities of the animal (hunting) and its urban or rural housing.

Prevention involves careful monitoring of dogs when returning from walks in the summer season and prompt conduct at the veterinarian in the event of signs suggestive of a spikelet.

In Ajaccio, dogs trained to detect a possible odor from the Covid-19 – Insurance for dogs


Ajaccio (AFP) – Will Inti, Nash, Onda and five other dogs become formidable agents in the fight against the Covid-19? Since Friday, these animals have been trained in Ajaccio to detect a possible odor of the virus, as part of a test conducted by veterinarians and firefighters.

Called Nosais, this research developed by Professor Dominique Grandjean of the National Veterinary School of Alfort, in the Paris region, plans to experiment with cynotechnical skills in order to identify particular odors that could be emitted by patients positive for coronavirus.

« If we can validate this experiment, the goal is to provide a complementary solution to the tests that already exist » to screen for the disease, explains to AFP Aymeric Bernard, chief veterinarian and cynotechnical advisor of the Fire and Rescue Service of Corse-du-Sud (SIS 2A), which takes part in the project.

« The hospital needs numerous and reliable means of screening. Today, the PCR test has a reliability of 70%. We therefore need to cross this test with other types of screening », indicates for his part the director of Ajaccio hospital, Jean-Luc Pesce, insisting on the interest of this experiment for his establishment.

Corsica was one of the regions most affected by the coronavirus epidemic.

With the support of the prefecture of Corse-du-Sud, the Regional Health Agency of Corsica, and the Ajaccian hospitals of La Misericorde and Eugenie, six shepherds from Mechelen and a dog of the Cursinu breed from the Corse-du-Sud firefighters are therefore tested on the site of a future veterinary clinic.

« These dogs are usually used to search for missing or buried people under a collapse, » says Aymeric Benard.

A Malinois shepherd of the gendarmerie, « used within the framework of research of specific products » and therefore already educated in « odorology », joined the team, he specifies.

– Dogs detecting other diseases –

Ajaccian hospitals will provide around 50 compresses placed for a few minutes under the armpits of patients who test positive for the virus.

« The point is that at the sweat level there is no viral expression today which has been noted and therefore a priori no risk of contamination, which means that the dog will be able to intervene in a secure manner « , specifies the SIS veterinarian.

Inserted in sterile jars, they will be placed in a small hatch, near the dog’s favorite toy.

Next to his master, the animal will then breathe this smell before retrieving his toy, and will therefore associate it with the game.

Then, the jar will be installed in one of the supports in the detection room. The goal for the dog will then be to « mark » by the sitting or lying position the support containing the positive compress, before being rewarded with his toy. A process that can be repeated fifty times a day for the next few weeks.

In the second phase, the dogs will be tested.

At the same time, the University of Corte (Haute-Corse), associated with the project, will work on the scientific validation of the protocol to determine on a cynotechnical level whether dogs can detect the smell of Covid-19.

« In addition, we want to check whether, using gas chromatography and mass spectrometry laboratory techniques, we can identify one or more compounds that can only be found on Covid positives », reveals Aymeric Benard.

The dog is already used for the detection of several chronic diseases, certain cancers, malaria, or even Parkinson’s disease.

« Lately the Americans have also used the dog in herds of cattle for the detection of a viral disease and this has given very good results, » said Aymeric Benard.

The first results of this Nosais trial will be known in mid-May. After Corsica, other territories should join the experiment in the coming weeks, such as the fire and rescue service of Seine-et-Marne or the battalion of sailors and firefighters from Marseille.

The same Nosais trial is launched in Lebanon under the aegis of the Franco-Lebanese University and similar projects exist in Germany, Canada, Norway, the United Kingdom and the United Arab Emirates, according to the initiators of the experiment.

« The samples are placed in sterile jars then in a small hatch, near the dog’s favorite toy. Accompanied by his master, he comes to breathe this smell before recovering his toy », describes Dominique Grandjean at our colleagues. Toy, because he has to choose a game and because the success of the dog will be sanctioned by a reward.

The first eight dogs are trained, by the Ajaccio firefighters, at the rate of a hundred presentations, each day, of pads impregnated with the sweat of patients. One more smell to memorize for animals that have several dozen in memory, for their work, nothing insurmountable according to the national veterinary school.

Research in Great Britain and the United Kingdom

The French, who are also associated with a Lebanese university, are not the only ones to work on this hypothesis. In Great Britain, Medical Detection Dogs, an association, pilots with two research and teaching institutions research of the same type, imagining after six to eight weeks of training having operational dogs. And being able to use them « to detect Covid-19 in airports at the end of the epidemic to quickly identify people carrying the virus ».

It is time to plan for the management of the next epidemic. No. It is not too early to address this issue now, although the Covid-19 pandemic is still at an early stage. The SARS (Severe Acute Respiratory Syndrome) epidemic, which struck the world in 2003, has given rise to no reflection on the management and prevention of the next health crisis. While it would have been possible to avoid the current Covid-19 epidemic, we are now paying the price for this failure to anticipate, which allowed the virus to spread by very probably following the same path as the Sras.

Emerging human diseases, whether Covid-19 and SARS but also AIDS, Ebola and Marburg, do not occur in humans spontaneously. Originally, these are animal diseases (also called zoonoses) which are transmitted from animals to humans. These diseases come from mammals, the closest animal class to humans.

While it would have been possible to avoid the current Covid-19 epidemic, today we are paying the price for this anticipation failure.

The reason for this mode of transmission is simple: a microbe evolves by adapting to the internal chemical environment of its host. If the internal chemical environment of its new host is comparable to that of its old host, the microbe will be more easily transmitted. Humans are also mammals, so most of these zoonoses are more widely transmissible.

Transmission of SARS to humans has occurred in Chinese wild animal markets. Indeed, everywhere in China, there are many businesses of this type, where wild animals which have been killed or captured are sold, dead or alive, for food or medicinal purposes. The SARS epidemic originated in this small carnivorous mammal, the civet, which had itself been contaminated through the bat. However, if most people should never have been in contact with this wild animal, the commercial activity of hunters consists in reselling civets and other animal species on the stalls of these markets.

If an evil alien wanted to develop the most effective method of infecting humans with zoonoses, he would try to maximize his chances by bringing as many mammalian species as possible into contact with as many humans as possible. And what would this miraculous bias be? A Chinese market for wild animals!

Annoyingly, the stalls of these shops abound in animal species on which customers get supplies, all likely to be candidates for infection. Of course, these wildlife markets are not limited to China. However, Chinese businesses have the difference that they are formidable places conducive to the outbreak of an epidemic. Indeed, China has the highest population in the world, increasingly connected to the rest of the planet by high-speed trains, planes, cars.

The SARS epidemic in 2003 should have raised awareness

These facts concerning both the animal origin of these emerging human diseases and the ideal mode of transmission, favored by these wildlife trade, have been known to public health workers for many years. The SARS epidemic that entered these markets in November 2002 should have led to an awareness of China and the closure of these sites. She kept them open.

When Covid-19 first appeared in Wuhan in December 2019, the metropolitan market was quickly suspected of being at the heart of the virus. At the moment, although we don’t have the evidence to confirm this hypothesis, everything suggests that. Indeed, Covid-19 is caused by a coronavirus which is very similar to the two previous epidemics of zoonotic coronaviruses, Sras and Mers, the common source of which originates in bats.

After the emergence of the Covid-19, the Chinese government initially reacted by seeking to minimize its importance, before taking a 180-degree turn and implementing a drastic package of measures of a scale never seen in the past. It appears that these measures have proven to be effective in stemming the epidemic. In addition, China has also decided to prevent the emergence of other zoonoses by finally ending the food trade in wild animals.

This is the good news. But there is also bad news. The Chinese government has not banned all possible routes of transmission of an infectious disease from wild animals to humans. I am referring to the animal market at the center of traditional Chinese medicinal practices. This is another heavy trade, very busy and in which many animal species also accumulate. For example, the pangolin, a scaly anteater, is used extensively in traditional medicine. Popular belief is that its consumption helps fight fever, skin infections and venereal diseases. However, whether for food or medicinal purposes, the microbe makes no distinction between these two markets and can infect anyone who visits one of them.

Until the wildlife trade stops, the next epidemic is only a matter of time

For Westerners, the closure of these infectious foci is obvious. In fact, the question that arises is: why the Chinese government, whose authoritarian governance allowed it to quickly impose drastic measures on its millions of nationals, does it not have the will to put an end definitely to animal trafficking? The explanation is that animal products are more than just a delicacy for some Chinese populations. By comparison, if scientists discovered that wine and cheese regularly caused epidemics, how would the French react if it were necessary to ban trade in these products? In China, the consumption of wild animals is an integral part of the culture, and even much more than that of cheese and wine in France.

On the other hand, as long as this trade in wild animals does not end definitively, in China as elsewhere, we are confident that another global epidemic will follow those of Sras and Covid-19. We have been relatively spared by SARS since the toll has remained below 1,000 deaths, much less than that of the seasonal flu responsible for hundreds of thousands of deaths each year. However, we won’t get away with the Covid-19 so well. Whether it causes more or less deaths than the seasonal flu, it will inevitably have a dramatic impact on the lives and work of millions and even billions of people around the world.

And the consequences of the next virus could be even worse. Growing globalization is increasing the flow of people and trade. Biologically, there is therefore no reason why future epidemics should not cause hundreds of millions of deaths and a depression unprecedented in history, which has lasted for several years.

Such a risk would be considerably reduced by ending the markets for the sale of wild animals for food and traditional medicine. This is not a favor from the Chinese government to the rest of the world, but a measure beneficial to the Chinese people themselves. As in the case of Covid-19, they will probably be the first victims of the next virus that will emerge from these businesses.

The exact origin of the new coronavirus, which has already infected nearly four million people worldwide, is not yet known with certainty. But most scientists agree that it is an animal virus. Bats, for example, are a natural reservoir for many coronaviruses. Captured and sold alive on a market in the Wuhan region, they could have directly infected patient zero in the epidemic.

The case of the coronavirus is not isolated. The MERS virus in the Middle East, Ebola, HIV, etc. There are many examples of epidemics from an animal that have ravaged human populations. A better knowledge of these infections could facilitate the prevention and the fight against these diseases. . What is a zoonosis?

The definition is apparently simple: it is an infectious agent of animal origin which is transmitted to humans, and which can make them sick. In reality, the definition evolves over time, or according to the research fields, explains Antoine Gessain, virologist at the Pasteur Institute, to the « Echoes ». It includes viruses, such as the new coronavirus, but also infections of bacterial or parasitic origin.

Scientists estimate that about two-thirds of the emerging viruses are of animal origin. It is especially mammals, which are closer to humans, who are at risk of transmitting these infectious agents to it. And it is generally wild animals which are at the origin of these epidemics.

. Besides the coronavirus, what other examples of zoonoses?

The new coronavirus, which paralyzes much of the global economy, is only the latest in a long history. It is not even, for the moment, the most deadly virus. Certain diseases of zoonotic origin thus persist in several regions of the world, even if they are transmitted very little from human to human. This is the case, for example, of rabies, which has emerged in wild carnivores and is transmitted each year to millions of people around the world, most often by stray dogs.

Two other coronaviruses have marked recent history. The first version of SARS (which stands for « severe acute respiratory syndrome ») emerged in China in late 2002 and killed nearly 800 people. And the MERS virus, which appeared in 2012 in the Middle East, continues to spread slowly in the region. The first comes from a bat and undoubtedly contaminated the man via a civet, sold on the markets of live animals in China. The second is probably transmitted to humans by dromedary.

This is also the case with Ebola, a virus which appeared in the 1970s in Sudan and the Democratic Republic of the Congo, and which continues to wreak havoc in Central Africa. Its natural reservoir is the bat. Particularly deadly, it was probably transmitted to hunters, before spreading to local populations.

Finally, the HIV epidemic, which has killed tens of millions of people around the world, also has a zoonotic origin. It is « probably blood contact between a hunter and a monkey », followed by a diffusion within human populations, which caused this pandemic, notes Antoine Gessain.

. Are these epidemics more and more frequent?

This is « probably » the case, notes the specialist from the Institut Pasteur. « Knowledge of these epidemics and the dissemination of information are much better, which leads to observation bias, » he observes. « But humans also have more and more contact with wildlife, » which increases the chances of contamination. « The data show that the number of epidemics is increasing despite this bias. »

Deforestation, for example to produce palm oil, leads to a loss of the natural habitat of animals such as bats, which serve as reservoirs for many viruses. Hunting and the consumption of bushmeat are all opportunities to come into contact with contaminated animals. Finally, large gatherings of people such as refugee camps are ideal grounds for the spread of an epidemic.

4. How to avoid them?

It is particularly difficult to anticipate the emergence of a new epidemic. What to do to protect yourself? For Etienne Decroly, a CNRS researcher specializing in emerging viruses, the fight against new epidemics requires, above all, better long-term funding for scientific research. “Veterinary surveillance”, in particular, provides a better understanding of new diseases. The risk is that interest will drop as soon as an epidemic is brought under control, as was the case for the coronavirus after the reflux of MERS and SARS-Cov-1.

The current epidemic has nevertheless shown the responsiveness of the scientific community, which has made it possible to sequence the virus in record time, nuance Antoine Gessain. “Better knowledge of the infectious agent promotes control of the epidemic. We already know that dogs have the ability to smell certain diseases. We also know that some viruses do have a specific odor. This is what has been discovered by teams at Auburn University in Alabama, who have demonstrated the ability of dogs to detect mucosal disease in cattle, a virus for which there is no there was no reliable screening test. We therefore wanted to reproduce this for the detection of SARS-Cov2. This research is part of the Nosais trial, a development project for medical detection by dogs. We have just launched the first tests, in which the Franco-Lebanese University of Beirut, the Fire and Rescue Service of Corse-du-Sud and the Seine-et-Marne fire brigade (SDIS 77) are participating ). The goal is to do the same thing as the Americans, but with the Covid-19, by banking on the fact that it leaves a specific trace in the organism, a trace that the scent of dogs can detect.

In the protocol you just launched, how do you train dogs to detect Covid-19?

A person with the coronavirus will eliminate traces of it, these are « catabolites » induced by the virus, which are found in urine, stool, tears, or saliva and sweat. Other countries have launched studies similar to ours, including urine or saliva, but the health risk in this case is higher.

Our test is about detecting the virus in sweat, because in sweat there is no virus shedding, so there is no risk of contamination. Especially since sweat characterizes each person, so if the test works, it can provide a very reliable detection mode.

In practice, we collaborate with hospitals, where axillary sweat samples are taken – under the armpits – from patients tested positive for coronavirus. Cotton swabs are placed for a few minutes under their arms, before being locked in airtight boxes for 24 hours to ensure that the samples no longer contain any virus particles. We work with fire dogs used to looking for missing persons, and with dogs from the company Diagnose, whose animals, trained in the detection of explosives in particular, have already integrated a catalog of dozens of specific odors. Here, it’s about training them to identify the characteristic smell that the Covid-19 might have. Or one more smell to the library of specific smells they have in memory.

The samples are placed in sterile jars and then in a small hatch near the dog’s favorite toy. Accompanied by his master, he comes to breathe this smell before recovering his toy. For him, smelling the virus becomes a game. Then, we submit a line of samples – one positive and several negative – to the dog’s flair, which will sit in front of the positive sample. Today, we are in this phase of training dogs to detect the smell of coronavirus. We started just a few days ago, but according to the first feedback we have, we can be sure that the coronavirus generates a particular fragrance that we find in sweat.

If this trial is successful, what could be the practical applications? How can this discovery be included in the French Covid-19 screening strategy? Have other countries already expressed interest in your work?

The first tests are encouraging, and we should have concrete results within three weeks. Then, it will be necessary to obtain the scientific validation of this protocol. If this virus detection method works, this will allow mass detection of Covid-19. A single dog could « test » several hundred people each day. This could be deployed at airports, on disembarkation of ocean liners, and even across certain municipalities. This would complement PCR and serological tests, which are not 100% reliable, which have a high rate of false negatives.

It is a fast device, which requires no sampling equipment, so no risk of shortage of reagent or swab! It is practical and inexpensive, which also makes it an interesting asset for the screening strategy of the poorest countries, which do not have a significant capacity for biological tests.

« The hospital needs many and reliable means of screening. Today, the PCR test has a reliability of 70%. We therefore need to cross this test with other types of screening », indicates for his part the director of Ajaccio hospital, Jean-Luc Pesce, insisting on the interest of this experiment for his establishment.

With the support of the prefecture of Corse-du-Sud, the Regional Health Agency of Corsica, and the Ajaccian hospitals of La Misericorde and Eugenie, six shepherds from Mechelen and a dog of the Cursinu breed from the Corse-du-Sud firefighters are therefore tested on the site of a future veterinary clinic.

« These dogs are usually used to look for missing or buried people under a collapse, » says Aymeric Benard. A Malinois shepherd of the gendarmerie, « used within the framework of research of specific products » and therefore already educated in « odorology », joined the team, he specifies.

Sniffing the sweat

Ajaccian hospitals will provide around 50 compresses placed for a few minutes in the armpits of patients who test positive for the virus. « The point is that at the sweat level there is no viral expression today which has been noted and therefore a priori no risk of contamination, which means that the dog will be able to intervene in a secure manner « , specifies the SIS veterinarian.

Inserted in sterile jars, they will be placed in a small hatch, near the dog’s favorite toy. Next to his master, the animal will then breathe this smell before retrieving his toy, and will therefore associate it with the game.

Then, the jar will be installed in one of the supports in the detection room. The goal for the dog will then be to « mark » by the sitting or lying position the support containing the positive compress, before being rewarded with his toy. A process that can be repeated fifty times a day for the next few weeks. In the second phase, the dogs will be put to the test.

At the same time, the University of Corte (Haute-Corse), associated with the project, will work on the scientific validation of the protocol to find out at the cynotechnical level if dogs can detect the smell of Covid-19.

In addition, we would like to check whether, by means of gas chromatography and mass spectrometry laboratory techniques, we can identify one or more compounds that are found only on Covid positives « , reveals Aymeric Benard.

First results known in mid-May

The dog is already used for the detection of several chronic diseases, certain cancers, malaria, or even Parkinson’s disease.

« Lately, the Americans have also used the dog within herds of cattle for the detection of a viral disease and that gave very good results », indicates Aymeric Benard.

The first results of this Nosais trial will be known in mid-May. After Corsica, other territories should join the experience in the coming weeks, such as the fire and rescue service of Seine-et-Marne or the battalion of marine firefighters in Marseille.

The same Nosais trial is launched in Lebanon under the aegis of the Franco-Lebanese University and similar projects exist in Germany, Canada, Norway, the United Kingdom and the United Arab Emirates, according to the initiators of the experiment.
